In the Spain-U.S. game, a player for the latter had her bathing suit grabbed and yanked underwater, resulting in a nipple slip - or full-on breast - on live TV:

This water polo player's wardrobe didn't really malfunction ... it was just forcibly removed.
Good times. As NBC's admittedly awesome underwater cameras aired the
intense action, they no doubt gave perceptive viewers more than they
bargained for, and guaranteed themselves a nice fine from the FCC.
